Meanwhile, a manic Ian plans his revenge, with Mickey and Mandy, against a homophobic military funeral protest. It was an interesting scene when the Wallace's and Norbert came out to the car and met Kev and V. Ian is feeling lifeless and turns to extreme measures to feel again. Mickey's alcoholic denial comes to an end, and he and Ian reunite. Bianca conquers her fears of stepping on train tracks tenfold when she and Frank sleep together on them, and the train almost crushes them.
Frank realizes that he's not ready to die along with Bianca. Frank, however, returns to put up a fight. Ian is admitted at the psychological evaluation ward. While undergoing treatment at the hospital, Frank's doctor, Bianca, reveals she has terminal cancer. Ian flees, with Mickey's baby in tow.

Veronica and Kev have relationship issues. At the Milkovich house, Ian helps a pregnant Svetlana take care of baby Yevgeny.
When Bianca said goodbye to Frank, I thought it was for good. Monica and Ian's road trip ends up at her meth dealing teenage boyfriend's property. Lip is met with financial aid difficulties, and his family is responsible as they carelessly left his mail sitting in a window. Meanwhile, Fiona preps an uncooperative Carl for his sentencing hearing. He later enlightens Sammi concerning his military insubordination. Homeless and penniless once again, Frank takes advantage of his liver donor's parents.
But when his business venture fails, Ian finds he has nowhere to turn. Fiona enters into an emotional crisis over Gus and Jimmy. But manual labor turns out to be harder than Lip thought and the brutal hazing from his co-workers doesn't help. This entire scenario is still fairly selfish because he wants to keep her around because he is in love with her, but his selfishness leads to actually trying to help her. Mickey drugs her, and she lays on the couch, wide eyed and seemingly dead as a doornail.
We were served two big shockers, which will most likely be addressed in the finale. He's the only one who can help. Ian returns home from the psych ward but refuses to accept his bipolar diagnosis or take his medication; Gus tells Fiona he is going on tour and that they need time apart; Frank recovers from his gunshot wound and plots revenge against Sammi; Carl is asked by his boss to run drugs to Indiana; Lip convinces Kev to sell drugs in the dorms. Meanwhile, Carl is asked by his boss to run drugs to Indiana; Frank sees a way to get revenge on Sammi by using Chuckie as the mule and having him busted; and Lip, still in need of tuition money, convinces Kev to come to the dorms and sell drugs. As the reality of his situation dawns, he realises he has no choice. Fiona, the eldest daughter, takes the role of the parent to her five brothers and sisters.
Part of me likes the dynamic between Frank and Bianca, but another part of me just can't get invested in this. After finishing his first year in college, Lip applies for a demolition job. Things are complicated even further for Fiona when she finds Jimmy sitting at one of her tables. Then, after a heartfelt plea, Lip is able to bide time concerning his financial woes at school. Fiona is rocked when a colleague overdoses on heroin; combined with her increasing worry that her recent decision to marry might have been a hasty one. Plus, Lip sleeps with a professor on the assumption he's only days away from being kicked out, Ian realizes it's time to get back on his meds and Kev finds himself enjoying college life as campus 'rapewalker.
Debbie continues to struggle with the pains of adolescence and Carl begins a new business venture as a drug-dealer. Meanwhile, Fiona and Gus continue to hit if off, so much so that Gus admits he's falling in love with her.
